Supported Community Living

Hourly ID/BI Waiver & Home Based Habilitation Services
 

These Medicaid programs are designed to support and encourage individuals to live as independently as possible in their own homes and in the community they desire. Staff is available to provide individualized hourly services at times that are convenient to the individual served. The goal of the program is to help the individuals learn various skills each of us acquire to live on our own, so individuals feel more self-sufficient and have more freedom with their choices. Premier also wants individuals to increase their involvement in the community.

The program provides training to help people develop or regain skills of daily living such as:

​

  • Home management skills

  • Social and leisure skills

  • Medical support services

  • Health and wellness education

  • Managing mental health symptoms

  • Financial services

  • Life management/independent living

  • Crisis management and problem solving

  • Apartment maintenance

  • Development of natural support systems

  • Transportation coordination

  • Advocacy
